A notarized document is one where a notary public has verified your identity, watched you sign (or confirmed that you signed), and added an official notarial certificate. Notarization does not make a document true or legal. It proves who signed it and that they did so willingly.
What a notarized document looks like
Your document with the notary section completed. In New York that includes the venue (state and county), the date, the notary’s signature, and the notary’s printed name, the words “Notary Public, State of New York,” registration number, county of qualification and commission expiration date. Most notaries use a stamp or seal to show those details cleanly.
Does an affidavit need a notarial seal?
It needs the notarial certificate with the required details. In practice that means the notary’s stamp. Our guide on seal versus stamp explains what New York requires. A missing or unreadable stamp is one of the top reasons a notarized affidavit gets bounced.
Why some forms ask the notary to circle something
Some forms print choices like “sworn / affirmed” or “personally known / proved by ID.” The notary marks the one that applies, usually by circling it or crossing out the other. You should not circle these yourself, and you should not leave them blank. If the form needs a choice and the notary does not make one, the receiving office may reject it.
The two main types
An acknowledgment confirms that you signed willingly (deeds, powers of attorney, contracts). A jurat confirms that you swore the contents are true (affidavits, sworn statements). The notary uses the wording that the document calls for, and cannot choose for you.

Frequently asked questions
What does it mean for a form to be notarized?
A notary verified your identity, witnessed or confirmed your signature, and added an official certificate. It proves who signed, not that the contents are true.
Does an affidavit need a notarial seal?
It needs the notary certificate with the notary details, which is normally shown with a stamp or seal. A missing or unreadable one can get the document rejected.
Do you circle anything on an affidavit before notarizing?
Only if the form gives options, such as sworn or affirmed. The notary marks the correct one. Do not mark it yourself or leave it blank.
What does a notary document look like?
Your document plus a completed notary section with venue, date, signature and stamp.
Can a document be notarized if I already signed it?
For a jurat or sworn statement, no. You have to sign in front of the notary. For an acknowledgment you may have signed earlier, but you must confirm the signature to the notary.
