The Affidavit of No Social Security Number is a New York State Department of Motor Vehicles (DMV) form (form NSS-1A). It is used when someone, often applying for a driver’s license or non-driver ID, has never been issued a Social Security number and needs to confirm that in writing to the DMV. Because it is a sworn statement, it must be signed in front of a notary to be valid. This form is specific to New York State; if you need it for a different state’s DMV, check with that state’s agency first.
