A One and the Same Person Affidavit (also called a name affidavit) is a sworn statement confirming that two different names on record, such as a maiden name and a married name, a misspelled name, or a nickname, all refer to the same person. It is commonly requested by banks, title companies, schools, and government agencies when records do not match exactly.

Yes. It is a sworn statement, so it must be signed in front of a notary to be valid. That is why paying takes you straight into your online notarization session.

What is the difference between a same person affidavit and a legal name change?

A same person affidavit simply confirms that two existing names refer to the same individual, it does not change your legal name. If you need to legally change your name, that requires a separate court process.

Will this fix my ID or records permanently?

It is meant to resolve a specific mismatch for the party requesting it, such as a bank or title company. It does not automatically update your ID, passport, or other official records.

What documents should I bring to support the affidavit?

Anything showing both names, such as a marriage certificate, divorce decree, or court order, is helpful to have on hand, though the affidavit itself is your sworn statement.

Can I use this for a maiden name and married name?

Yes, that is one of the most common uses, confirming that your maiden name and married name refer to the same person.

Can I download the notarized document right after my session?
